JDS 2019-2020 Yearbook Dancer Portraits

Joni’s Dance Studio will once again create a beautiful full color yearbook. The
yearbook will include class candids and class pictures of all of our dancers.
Every dancer participating in the 2020 recital will receive a yearbook.

All JDS families have the opportunity to showcase their dancer with a
personalized “Dancer Page”. Dancer pages are full page- 8.5x11- and full
color and will be included in the yearbook. (For families with siblings also enrolled at
JDS, there is the option to create a dancer page with the siblings together for the same price.)

With purchase of a dancer portrait you will receive:
• Personalized on location portrait session with Studio Benton
• Action and posed dance shots
• Design consultation with Studio Benton to create the ultimate dancer page for
your dancer

**BONUS OFFER**: For any dancer who registers for a dancer page
before April 1st and schedules the portraits on or before April 24th will have a
free poster size dancer page displayed in the foyer of the recital auditorium.
Dancers will be able to take these home after recital.

Dancer pages are $85/page

and valuable consideration, the receipt of which is hereby acknowledged, hereby irrevocably authorize Studio Benton and Jonis Dance Studio to use photographs of me and or my property and authorize him/her/their and his/her/their assignees, licensees, legal representatives and transferees to use and publish (with or without my name, company name, or with a fictitious name) photographs, pictures, portraits or images herein described in any and all forms and media and in all manners including composite images or distorted representations, and the purposes of publicity, illustration, commercial art, advertising, publishing (including publishing in electronic form on CDs or internet websites), for any product or services, or other lawful uses as may be determined by the photographer or studio name here. I further waive any and all rights to review or approve any uses of the images, any written copy or finished product.
I am the parent or legal guardian of the above mentioned minor (Dancer first name and last name) and have the legal right and authority to execute the above release on behalf of the minor.
